Total Pelvic Exenteration

چکیده
expanded in the last years. Published data has demonstrated the feasibility and safety of this technique for several several gynecologic oncology procedures. Main advantages when compared to open approach are less blood loss, decreased morbidity, shorter hospital stay, and earlier recovery. Tumor dissemination and port site implantation have been described for patients undergoing operative laparoscopy for resection of various carcinomas. However, rate of port-site metastasis after laparoscopic approach for cervical cancer is reported to be less than 1% (1) Total pelvic exenteration is one of the most mutilating surgical procedures performed for pelvic malignancies. Since it was first reported in the 50 ́s, postoperative mortality and long-term survival have improved but associated complications occur in approximately half of the patients. New surgical strategies have to be made to decrease morbidity and improve patients’ quality of life (2). Laparoscopic approach for pelvic exenteration was first described in 2003 and since then, several series have reported on technical feasibility of the procedure (3).

Complications and Outcome of Pelvic Exenteration in Thai Gynecologic Oncology Patients
Pelvic exenteration is a radical surgery which includes the en bloc resection of the pelvic organs and followed by surgical reconstruction (Hockel et al., 2006; Diver et al., 2012). The en bloc resection includes internal reproductive organs, rectosigmoid colon, urinary bladder and distal ureters. Pelvic exenteration is classified into 3 types; anterior pelvic exenteration, posterior pelvic exe...

•Pelvic exenteration can be used in patients with multifocal recurrence.•Ability to achieve negative margins remains a necessity for pelvic exenteration.•Individualized treatments are essential for those with recurrent malignancy.
متن کاملRobotic Total Pelvic Exenteration with Laparoscopic Rectus Flap: Initial Experience
Total pelvic exenteration is a highly morbid procedure performed for locally advanced pelvic malignancies. We describe our experience with three patients who underwent robotic total pelvic exenteration with laparoscopic rectus flap and compare perioperative characteristics to our open experience. Demographic, tumor, operative, and perioperative factors were examined with descriptive statistics ...

•A multi-layer technique for reconstruction after pelvic exenteration is proposed.•Human acellular dermal matrix used in reconstruction after total pelvic exenteration.•A reconstructive technique based on human dermis, omental flap and fat is proposed.
متن کاملLong-term survival after total pelvic exenteration in a patient with recurrent cervical carcinoma: A case report
The management of recurrent cervical cancer depends mainly on previous treatment as well as on the site and extent of recurrence. Pelvic exenteration usually represents the only therapeutic approach with curative intent for women with central pelvic relapse who have previously received irradiation. In the present report, we share our experience regarding survival outcome in a patient with recur...
